1. Membership Eligibility

2. Membership Validity

All membership plans are valid for 12 months from the date of activation unless otherwise stated.

3. Membership Benefits

• Benefits are available only during the active membership period.
• Benefits vary according to the selected plan.
• Unused consultations, discounts or tests cannot be carried forward.
• Benefits are non-transferable unless specifically mentioned.

4. Consultation Policy

• Doctor consultations are subject to clinic working hours.
• Appointments should be booked in advance.
• Emergency services are not included under the membership.
• Online consultations may be offered based on availability.

5. Diagnostic Tests

Complimentary diagnostic tests included in the membership must be availed during the membership period. Tests are limited to those specifically mentioned in the selected plan.

6. Medicine & Investigation Discounts

• Discounts are applicable only at participating facilities.
• Discounts cannot be combined with other promotional offers.
• Discount percentages may vary depending on products or services.

7. Family Coverage

Family coverage, where applicable, is limited to immediate family members as defined by the clinic. Supporting identification may be required.

8. Membership Fees

• Membership fees are payable in advance.
• Fees are non-refundable after activation.
• The clinic reserves the right to revise pricing for future memberships.

9. Cancellation & Refund Policy

Membership once activated cannot be cancelled or refunded. Exceptional cases may be considered solely at the discretion of the clinic management.

10. Limitation of Services

Membership plans are preventive healthcare programs and do not replace hospitalization, emergency care, surgery, or specialist treatments unless explicitly mentioned.

11. Changes to Membership Plans

The clinic reserves the right to modify membership benefits, policies, pricing or terms at any time. Existing members will continue receiving benefits applicable to their active membership unless otherwise notified.

12. Privacy

All patient information will be maintained confidentially in accordance with applicable healthcare privacy practices.

13. Acceptance of Terms

By purchasing a Health Membership Plan, you acknowledge that you have read, understood and agreed to these Terms & Conditions.
